Request to Record | Patient Portal Dashboard Animation
The patient side of Pulse Clinic, a clinic management system I designed and shipped for small
practices. Two screens, and the two rules they print about themselves.
Appointments says booking sends a request and the clinic confirms the slot, so rescheduling here
cannot produce a new time. It produces a pending one: Thursday 7 August, 09:40, marked Requested,
with Cancel request where Reschedule was.
Health record says reading it is itself recorded on the audit log, so opening the page writes an
entry the patient can see. The rest is provenance. Type 2 diabetes is why the review exists, and
the vitals are dated 18 July because that is the follow-up they came from.
Allergies read as recorded none, rather than never asked. That distinction is the design.
